If you use T588M and the respective feature in standard, you'll have the standard screen for your country. There may not be a special screen for UAE.
BUT: did you not check whether it works correctly, if you capture the form of address correctly as Mr. Ms etc?
If you don't need anything other than male or female in the gender field you should NOT need the gender field on the screen, as it's filled automatically from the form of address field.
