Here are some observations
1. The BOM Main material will be repeated as many times as it was used in the hierarchy
2. But the BOM Component materials (of the above repetitions) will not be repeated in the output.
3. If the BOM main material is used as a Construction type in the Equipment master, it will not be displayed. (P-1000 in your case).
Apply these observations to your case. With these observations the output seems to be OK for me.

One more thing:
You used the word Equipment BOM in your post. No Equipment BOM is seen in your picture. P-1000 is a Material BOM used as Equipment BOM using Construction Type field of Equipment master.
